Introduction: Difficult intubation is an emergency situation that anaesthetist come across frequently. It a frequent cause of mortality in practice anesthesia. Mallampati classification (MPC) the oropharyngeal structures simple test to assess anticipated difficult airway for endotracheal intubation. thyromental height (TMH) now also commonly used predict Objective: To evaluate diagnostic accuracy modified mallampati and using Cormack Lehane's laryngoscopy as gold standard. Study Design: Descriptive cross sectional validation study. Setting: Department Anaesthesia, Benazir Bhutto Hospital, Rawalpindi. Duration: 20-Sep-2021 19-Mar-2022. Material Methods: A total number 1035 were included this Pre-operatively (MMT) TMH was performed. class III IV considered difficult. While >50 mm equipment’s ready, i.e. stylet, boogie, LMA, I-gel. There 768 (74.20%) males 267 (25.80%) females. 77.8% sensitive, 94.6% specific having 70.0% PPV, 96.3% NPV 92.27% accuracy. On MMP taking according standard standard, 92.4% 90.1% 60.2% 98.6% 90.3% Practical implications: Thyromental Height needs more broad usage multiple settings be validated single most important predictor laryngoscopy. will thus widely all over country rendering fewer complications patients.
